Job Description

The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care. This role enhances the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The Case Manager I integrates and coordinates utilization management, patient education, care facilitation, and discharge planning functions, accountable for a designated patient caseload.

Responsibilities

• Facilitation of precertification and payor authorization processes.
• Collaborative management of patient care across the continuum.
• Application of process improvement methodologies in evaluating outcomes of care.
• Support and coaching of clinical documentation efforts.
• Coordinating communication with physicians.

Required Experience / Certifications / Licensure

• Minimum 5 years acute inpatient case management experience.
• Experience in acute discharge planning.
• Bachelor of Science in Nursing (BSN) required.
• Current and valid license to practice as a Registered Nurse in the State of Michigan.
• BLS certification through AHA.
• Previous experience using CarePort system.
• Candidates with experience charting in AllScripts, GEMS/Sunrise are preferred.
