Job Description

We are seeking a dedicated and experienced Registered Nurse Case Manager to join our team. The role involves performing care coordination, discharge planning, and managing patient needs to ensure optimal outcomes. The ideal candidate will possess a strong understanding of acute medical care management, as well as experience in various case management settings.

Responsibilities

• Assessing patients and coordinating care effectively.
• Participating in family meetings and treatment plan development.
• Organizing discharge planning and care coordination activities.
• Arranging durable medical equipment (DME), skilled nursing facilities (SNF), and ambulance transport independently.
• Providing expertise in managing diverse patient needs, including mental health and substance abuse cases.

Required Experience / Certifications / Licensure

• Minimum 2 years of experience in acute medical care management and care coordination.
• Active California state RN license; pending licenses may be accepted if active by the start date.
• Trauma Level I or II experience is preferred.
• Proficient in Epic charting system is required.
• Must have prior travel experience.
