Job Description

We are seeking an experienced Registered Nurse specializing in Case Management. The ideal candidate will have a minimum of 2 years of acute medical Care Manager experience with a focus on Care Coordination. The role involves travel, requires a California state license, and entails participation in the development of treatment plans as well as discharge planning.

Responsibilities

• Assess patients and manage care coordination.
• Organize and participate in family meetings.
• Develop treatment plans collaboratively.
• Experience with HMOs, IPAs, and relevant regulations.
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute medical Care Manager experience.
• California state license required; pending license accepted if licensed upon start date.
• Experience with trauma level I or II facilities preferred.
• Proficiency in Epic charting system is required.
• Travel experience is mandatory.
