Job Description

We are seeking a Licensed Practical Nurse (LPN) to join our team at Woodbridge Rehab. & Nursing. The successful candidate will provide high-quality patient care, working closely with other nursing staff to ensure all patient needs are met. The role requires floating to different units as needed and comfortable interaction with dementia patients.

Responsibilities

• Provide nursing care to patients, including monitoring vital signs and administering medications.
• Assist in the daily living activities of patients within assigned units.
• Work collaboratively with healthcare team members to develop and implement patient care plans.
• Ensure the comfort and safety of patients, particularly those with dementia.
• Maintain accurate patient records and documentation.

Required Experience / Certifications / Licensure

• A minimum of 3 years of LPN experience required.
• Prior travel nursing experience is required.
• Long-term care (LTC) experience is preferred.
• BLS certification through the American Heart Association required.
• Willingness to float to other units as needed.
• Ability to work effectively with dementia patients.
