Job Description

We are seeking a Home Health Registered Nurse to provide high-quality, patient-centered care in the Medford, OR area. The RN will be responsible for delivering nursing services in patients' homes, ensuring adherence to established guidelines and protocols. This position offers the chance to work one-on-one with patients while managing various aspects of their care, including assessments, medication management, and wound care.

Responsibilities

• Perform admission assessments, routine visits, and start of care for patients.
• Manage and administer medications using various methods, including intravenous and subcutaneous.
• Utilize pain scales for assessment and management, including PCA pump management and wound care.
• Document patient information and care in a computerized system (OASIS preferred).
• Ensure adherence to infection control precautions and appropriate discharge planning.

Required Experience / Certifications / Licensure

• 1 year of skilled home health experience required.
• Current Basic Life Support (BLS) certification required.
• Experience with OASIS documentation preferred.
• Ability to work independently and manage multiple patient visits per day.
• Driver's license and reliable transportation required, with the ability to travel up to 50 miles for patient visits.
