Schedule

Day shift, 8 hours (08:00 AM - 04:30 PM) - 5 days a week. This facility cannot accommodate block schedules.

Job Description

The Home Health Registered Nurse will be responsible for providing skilled nursing care to patients in their homes. The RN will conduct assessments, develop care plans, and implement care based on individual patient needs. The ideal candidate will have experience in adult home health and be proficient in computerized documentation, specifically OASIS.

Responsibilities

• Perform admission assessments and routine visits
• Discharge patients when needed due to safety or non-compliance
• Manage medication administration and assessments
• Implement and monitor pain management strategies
• Provide wound care and follow protocols for infection control
• Utilize computerized documentation for patient care

Required Experience / Certifications / Licensure

• Minimum of 1 year experience in skilled adult home health
• BLS certification required
• OASIS experience preferred
• Strong experience with medication administration and management
• Experience with wound care and infection control precautions
