Case Manager - Day Shift - 13 Weeks

Job Description

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation. The role emphasizes patient and employee safety, and supports patients and families in adapting to their healthcare situations.

Responsibilities

  • Identify patients' problems affecting their ability to participate in their treatment plan.
  • Assist patients and families in utilizing hospital resources to remove barriers to healthcare adjustments.
  • Collaborate with the interdisciplinary team, including physicians, to manage clinical plans for safe discharges.
  • Interact appropriately with patients based on their age and understanding.
  • Refer patients to outside agencies for social issues as required by regulations.
  • Communicate major conditions or changes to senior management.
  • Ensure timely and thorough completion of all documentation and required reports.
  • Promote patient and employee safety through adherence to safety guidelines.
  • Facilitate team and family conferences according to policy.
  • Manage and document payment processes and ensure compliance with payor requirements.
  • Maintain an updated community resource system for patient and family access.
  • Identify unnecessary appointments or procedures in collaboration with the clinical team.
  • Support fiscal responsibilities by managing billing and reimbursement processes.

Required Experience / Certifications / Licensure

  • Current licensure in a clinical discipline per state guidelines (preferred: RN, RT, SW).
  • Associates Degree in a healthcare field required (Bachelor’s degree preferred).
  • Working knowledge of the insurance industry and government reimbursement required.
  • Minimum of 2 years’ experience in Case Management and Discharge Planning in an Acute Care or Rehab setting.
