Job Description

The Case Manager, a Registered Nurse, will be responsible for coordinating patient care, managing discharge planning, and ensuring all care delivery meets regulatory and evidence-based guidelines. The position will require working collaboratively with healthcare teams to assess patients' needs and implement comprehensive care plans.

Responsibilities

• Care coordination and discharge planning for acute medical patients.
• Assessing patient needs and determining medical necessity based on evidence-based guidelines.
• Performing needs assessments and ordering durable medical equipment (DME).
• Maintaining regulatory compliance with CMS, HIPAA, and Department of Health guidelines.
• Utilizing computerized documentation systems effectively (Epic required).

Required Experience / Certifications / Licensure

• 2 years of acute medical care management experience, specifically in care coordination and discharge planning.
• Experience in case management, utilization review, and disease management.
• Familiarity with CPT coding, DRG, and patient ratios relevant to acute care settings.
