Registered Nurse - Case Manager

This is a 13-week contract position.

Schedule

Day 5x8-Hour (08:00 - 16:30) with weekend coverage typically required every other weekend.

Job Description

We are seeking a dedicated Registered Nurse specializing in Case Management. The ideal candidate will have a strong foundation in care coordination and discharge planning within an acute hospital setting.

Responsibilities

• Coordinate patient care and discharge planning according to regulatory guidelines.
• Assess patient needs and develop a comprehensive plan of care.
• Utilize Epic for computerized documentation.
• Ensure compliance with evidence-based guidelines for medical necessity.
• Manage care for a caseload of up to 25 patients daily.

Required Experience / Certifications / Licensure

• 2 years of acute medical Care Manager experience focused on care coordination and discharge planning.
• Familiarity with CMS guidelines, CPT coding, and HIPAA regulations.
• Experience with HMOs, IPAs, and Disease Management preferred.
• Travel experience required; first-time travelers can be considered.
