Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, demonstrating knowledge of patient care appropriate to the age of the patients served. The role requires the ability to assess data reflective of the patient's status and the capability to provide the necessary care to adult and geriatric patient groups.

Responsibilities

• Collect ongoing data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and other interdisciplinary team members.
• Create and execute age-appropriate and culturally relevant plan of resident care with input from the resident/family and healthcare team.
• Recommend revisions to the care plan based on evaluation of outcomes and facilitate appropriate referrals.
• Maintain knowledge and competency in nursing practice and assist in mentoring/orientation of staff.
• Manage occupational exposure to blood borne pathogens in accordance with safety protocols.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure
• BLS Certification (through AHA or RQI)
• 1 year of experience as an LPN, preferably in a long-term care facility
