Physical Therapist Job Description

Day 5x8.0

Position Overview:

The Physical Therapist will evaluate all assigned patients, developing, implementing, and modifying treatment plans in accordance with patients' needs and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate an effective return to community activities. The Physical Therapist will also serve as an educator for patients, families, students, and coworkers.

Key Responsibilities:

• Complete a comprehensive evaluation of assigned patients to develop appropriate treatment plans and goals focused on progression toward discharge within an established length of stay.
• Collaborate with an interdisciplinary team to address barriers to discharge, including timely equipment trialing, completion of patient and family training, and planning for continuing services.
• Manage individual, concurrent, and group treatment sessions efficiently based on patient care needs.
• Document medical records clearly, accurately, and in compliance with state, federal, and hospital policies.
• Provide oversight and mentorship to students as assigned, fostering a positive learning environment.
• Develop and execute a personalized professional development plan in collaboration with clinical management, pursuing ongoing education and professional activities.
• Engage in patient and professional advocacy efforts through various community and professional activities.
• Adhere to discipline-specific State Practice Act and regulatory standards while maintaining professional licensure.
• Demonstrate awareness and participation in organizational performance improvement initiatives.

Qualifications:

• BLS Certification required prior to first shift.
• Must hold a valid, current State license.
• Prior experience in acute care and inpatient rehabilitation settings is required.
• Proficiency in EMR systems, specifically EPIC, is highly desired.
