Job Description: Home Hospice Registered Nurse (RN)

Shift

AM 8:00 AM - 4:00 PM

On-call shifts required including overnights and weekends: 1-2 per month.

Overview

We are seeking a dedicated and experienced Home Hospice Registered Nurse (RN) to join our team. The ideal candidate will have a minimum of three years of experience in a hospice care facility and possess a valid New Jersey RN license. This role is essential for providing high-quality patient care and support to patients and their families during challenging times.

Duties & Responsibilities

• Patient Assessments: Conduct thorough assessments to determine patient eligibility for hospice care and identify learning needs and goals.
• Plan of Care Development: Create and implement comprehensive care plans for patients and families.
• Service Utilization: Maintain productivity standards and demonstrate knowledge of insurance reimbursement processes.
• Discharge Assessments: Ensure timely completion of admission assessments and data transfers.
• Patient Education: Utilize age-appropriate teaching methods to educate patients and their families; evaluate and revise teaching plans as necessary.
• Case Management: Manage cases effectively, prioritizing care needs and revising plans based on evidence-based knowledge.
• Clinical Safety: Maintain a safe environment by adhering to infection control measures and medication management protocols.
• Interdisciplinary Collaboration: Participate in IDT meetings every two weeks and communicate patient progress with the team.
• Performance Improvement: Engage in data gathering for performance improvement initiatives and provide orientation/mentorship to new employees.
• Agency Growth: Contribute to the growth of the agency by accepting additional assignments and facilitating new staff orientation.

Qualifications

• Current New Jersey RN License.
• Minimum of 3 years of experience in home hospice care.
• Experience with electronic medical records (EMR).
• Excellent critical thinking and communication skills.
• Valid New Jersey driver's license and Basic Life Support (BLS) certification required.
