**Job Description: Skilled Nursing Facility (SNF) Rehabilitation Specialist** We are seeking an experienced Rehabilitation Specialist to join our team in a Skilled Nursing Facility (SNF). The ideal candidate will possess a minimum of one year of SNF experience and hold an active California license. Proficiency in NetHealth (Rehab Optima) is essential for this role. **Key Responsibilities:** - Provide high-quality rehabilitation services to patients in accordance with established clinical guidelines. - Collaborate effectively with interdisciplinary teams to develop and implement patient care plans. - Document patient progress and treatments accurately using electronic health record software, specifically NetHealth (Rehab Optima). - Ensure compliance with all regulatory standards and facility policies. - Support patient education and rehabilitation goals to optimize recovery and quality of life. **Qualifications:** - Minimum of one year of experience working in a Skilled Nursing Facility. - Active California state license. - Proficiency in using NetHealth (Rehab Optima) for documentation and patient management. - Strong communication and interpersonal skills. - Ability to work collaboratively in a fast-paced environment.
