**Job Description: Case Management RN** **Position Overview:** We are seeking a dedicated Registered Nurse (RN) for a Case Management position within an acute care facility. The successful candidate will coordinate patient care, focusing on discharge planning and care transitions across various units. This role involves collaborating with an interdisciplinary team to ensure that patients receive comprehensive care tailored to their individual needs. **Shift Details:** - **Type:** Day Shift (4x10 Hour Schedule: 07:30 AM - 06:00 PM) - **Start Date:** ASAP - **Floating Required:** Yes, within the same facility and potentially to a nearby campus **Key Responsibilities:** - Collaborate with case management teams to coordinate patient care and discharge planning. - Float between units such as Emergency Department (ED), Intensive Care Unit (ICU), Pediatric Units, and others as needed. - Complete needs assessments and care plans to facilitate a seamless transition for patients. - Utilize EPIC for charting and documentation of patient information. - Work closely with Social Services and other interdisciplinary team members to address patient needs. **Qualifications:** - Minimum of 1 year of nursing experience in an acute care setting. - Familiarity with care coordination and discharge planning processes. - Experience with EPIC charting preferred. - Effective communication and interpersonal skills. **Additional Information:** - The RN will be required to work weekends, typically every other weekend. - Candidates must be willing to float between various units within the facility and potentially to the Pacific Campus, located 1.7 miles from the primary location. - A Washington State RN license is required, and pending licenses may be accepted. **Patient Population:** - Age groups served include neonates, infants, adolescents, adults, and geriatrics. **Work Environment:** - The facility is a Level II Trauma Center with a total of 571 staffed beds across two campuses. - Attire is business casual, and parking costs are covered by the facility. --- At ARMStaffing we take care of our nurses!
