**Job Title: Case Management RN** **Shift: Day 4x10-Hour (07:30 AM - 06:00 PM)** **Start Date:** ASAP **Experience Required:** Minimum of 1 year **First-Timers Accepted:** Yes **Weekends Required:** Usually Every Other Weekend **License Required:** Valid WA RN License or Compact RN **Overview:** We are seeking a dedicated Case Management RN to join our team in a Level II Trauma Center. This role involves coordinating care within an acute care facility, partnering with a team of case managers to ensure effective patient discharge planning and care coordination. This position requires the ability to float between different units within the hospital based on patient care needs. **Key Responsibilities:** - Collaborate with interdisciplinary teams to develop and implement comprehensive care plans. - Conduct needs assessments and plan for equipment and services necessary for patient care. - Coordinate patient transitions and discharge plans for optimal outcomes. - Float to various units including ED, ICU, Pediatrics, and other specialty areas as required. - Participate in regular meetings and contribute to continuous improvement in care coordination processes. **Patient Demographics:** - Work with a diverse patient population, including neonates, adolescents, adults, pediatrics, and geriatrics. **Qualifications:** - Minimum of 1 year of experience in acute care or case management. - Experience with discharge planning and care coordination is essential. - Proficiency in EPIC charting system preferred. - Strong communication and collaboration skills. **Additional Information:** - All travelers will be expected to float as necessary between like units. - Candidates must provide all requests for time off (RTO) at the time of submission. - Orientation hours are provided prior to starting. **At ARMStaffing we take care of our nurses!
