**Job Description: Case Management RN** **Shift:** Day 4x10-Hour (07:30 - 18:00) **Start Date:** ASAP **Location:** Acute Care Facility **Experience Required:** Minimum 1 year (First-timers accepted) **Weekends Required:** Typically every other weekend **Certifications Required:** WA RN License (or Compact RN License) **Floating Required:** Yes, based on unit needs **Position Overview:** We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team in the Case Management department. In this role, you will work closely with a group of case managers to coordinate care within an acute care facility. The RN will need to demonstrate strong skills in discharge planning, care coordination, and effectively managing patient needs across various units. **Key Responsibilities:** - Coordinate and manage patient care from admission through discharge, ensuring a smooth transition of care. - Collaborate with multidisciplinary teams, including physicians, social workers, and other healthcare professionals to optimize patient outcomes. - Float between different units as required, including Emergency Department (ED), Intensive Care Unit (ICU), Pediatrics, and Specialty Units. - Perform needs assessments and develop individualized plans of care, addressing both medical and psychosocial aspects. - Maintain accurate and timely documentation in the electronic medical records system (EPIC). - Participate in quality improvement initiatives and ensure compliance with regulatory requirements. **Qualifications:** - Current Washington RN License or Compact RN License required. - Minimum of 1 year of experience in case management or related field. - Previous experience in acute care settings preferred. - Familiarity with EPIC charting system is highly desirable. - Strong interpersonal and communication skills enabling you to work effectively in multidisciplinary teams. **Additional Information:** - This position will require working in both the Colby Campus and, potentially, the Pacific Campus (approximately 1.7 miles apart). - Orientation hours will be provided for new hires, aimed at ensuring a seamless integration into our team. **Patient Demographics:** You will work with a diverse patient population including neonates, infants, children, adults, and geriatrics.
