**Job Title: Case Manager - Acute Care Facility** **Shift: Day 4x10-Hour (07:30 AM - 06:00 PM)** **Start Date: ASAP** **Experience Required: Minimum of 1 year** **First-timers Accepted: Yes** **Weekends Required: Typically every other weekend** **Floating Required: Yes, based on need** **Housing: No local candidates accepted** **Job Description:** We are seeking a dedicated Case Manager to join our dynamic healthcare team at a Level II Trauma Center. The successful candidate will play a vital role in coordinating patient care and facilitating effective discharge planning within an acute care setting. **Key Responsibilities:** - Collaborate with a team of case managers to oversee patient care coordination across multiple units within the hospital. - Navigate through a variety of units, including ED, ICU, Peds, and specialty units based on patient and facility needs. - Engage in comprehensive discharge planning to ensure a smooth transition of care for patients. - Work efficiently with the EPIC charting system to document patient interactions and care plans accurately. - Provide thorough assessment of patient needs, including ordering Durable Medical Equipment (DME) and developing individual plans of care. **Qualifications:** - Current Washington RN License (or Compact RN License). - Minimum of 1 year experience in case management, preferably in an acute care setting. - Experience working with diverse patient populations, including neonates, adolescents, adults, and geriatrics. - Proven background in care coordination and discharge planning. - Strong interpersonal skills, with the ability to work collaboratively in a team environment. - Must be willing to float between varying units as needed. **Additional Information:** - Attire: Business Casual - Parking: Facility Paid - Orientation: 30 hours of orientation provided - Scheduling Type: Block (with the expectation of working three of the five major holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, and New Year’s Day). **At ARMStaffing we take care of our nurses!
