**Job Title: Case Management RN - Acute Care Facility** **Shift:** Day Shift (5x8-Hour, 08:00 - 16:30) **Start Date:** ASAP **Experience Required:** Minimum 1 year case management experience **Float Requirement:** Yes, based on need **Certifications Required:** WA License **Call Requirement:** No **Local Candidates:** Not accepted **Pending License Accepted:** Yes **Job Description:** We are seeking a dedicated and experienced Case Management Registered Nurse (RN) to join our team in a fast-paced acute care facility. The successful candidate will collaborate with a team of case managers to coordinate care and manage discharge planning for patients within our unit. This position involves floating to different units within the facility as needed, including the possibility of transferring to a nearby campus. **Key Responsibilities:** - Work alongside case managers to coordinate comprehensive care for patients. - Collaborate with interdisciplinary teams to facilitate smooth transitions in patient care. - Conduct assessment and planning based on patient needs and care requirements. - Develop and implement discharge plans in compliance with healthcare regulations. - Utilize the EPIC charting system for documentation and patient management. - Maintain high standards of patient care across various units, including emergency department, ICU, pediatrics, and specialty units. **Qualifications:** - Minimum of 1 year of case management experience in an acute care setting. - Familiarity with discharge planning and care coordination processes. - Proficient in using electronic health record systems, preferably EPIC. - Ability to work independently and as part of a team in a dynamic environment. - Flexibility to float within the hospital and to sister facilities as required. - Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals. **Additional Information:** - Valid WA RN license required. - Applicants must provide two supervisory references covering at least one year of relevant experience within the last three years. - All travelers can expect to float between like units. **Patient Demographics:** - Experience working with diverse patient age groups including neonates, adolescents, adults, and geriatrics required. **At ARMStaffing we take care of our nurses!
