**Job Title: Home Health Nurse** **Shift: Day (5x8-Hour, 08:00 - 17:00)** **Job Description:** We are seeking a dedicated and skilled Home Health Nurse to provide high-quality medical care to patients in their homes. The ideal candidate will have substantial experience in infusion therapy, skilled home care, and home health services, with a focus on patient assessment and intervention in various medical conditions. **Responsibilities:** - Carry out comprehensive cardiopulmonary assessments and manage basic wound care. - Perform tasks such as ABI compression dressing, wound vac dressing, and Pleurx/Aseptdrain care. - Administer Foley catheter changes, staple/suture removal, and injection procedures (SQ/IM). - Conduct lab draws through venipuncture and provide IV skills, including the care and dressing change of PICC or Midline lines. - Utilize glucometers and pro-time machines for patient monitoring and management. - Offer home skilled visits, typically 6-8 per day, and adapt to varying shift hours as needed (Monday-Friday 8a-5p, 9a-6p, or alternating weekends). - Complete documentation using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems. **Qualifications:** - Must have a minimum of one year of relevant nursing experience with infusion and home health care; private duty nursing experience will not be accepted as a substitute. - Proficient in OASIS training and experienced with Medicare, Medicare Advantage, and Commercial insurance home care services. - Valid Florida license or compact license required upon application. - Must possess a valid driver’s license and have reliable transportation for home visits. - COVID-19 vaccination is required, or an exemption form must be on file. - Flu vaccination or declination necessary with mandatory mask-wearing while at work. - Recent patient care experience within the last 3 months is essential. - Preferred experience with Cerner charting system. **Work Environment:** - Work primarily involves home health visits, providing individualized care to adult and geriatric populations. - Collaborate with interdisciplinary teams including physical therapy, social services, and pharmacy support. **Special Requirements:** - Compliance documents must be submitted ten days prior to the start date. - Candidates must be flexible with their schedules and may be required to work alternating weekends.
