**Job Description: Case Manager (Clinical Liaison) - Registered Nurse/Occupational Therapist/Physical Therapist** We are seeking a dedicated and compassionate Case Manager to join our Acute Rehabilitation Unit. As a Clinical Liaison, you will play a crucial role in ensuring the optimal care and support for patients within our 28-bed unit. This position is essential for the effective management of patient transitions and resources, contributing to the overall success of our rehabilitation services. **Key Responsibilities:** - Conduct patient assessments to develop, implement, and update individualized care plans. - Collaborate with interdisciplinary teams to coordinate and facilitate patient care. - Act as a liaison between patients, families, and healthcare professionals to promote effective communication and continuity of care. - Manage referrals and discharge planning to ensure seamless transitions for patients. - Educate patients and their families on treatment plans, available resources, and follow-up care. - Maintain accurate documentation in compliance with regulatory standards and facility protocols. **Qualifications:** - Current state licensure as a Registered Nurse, Occupational Therapist, or Physical Therapist. - Proven experience in case management or a similar role, preferably in an acute rehabilitation setting. - Strong communication, organizational, and problem-solving skills. - Familiarity with Meditech or other electronic medical records (EMR) systems. - Ability to work collaboratively within a team and adapt to dynamic environments. **Work Schedule:** - Monday to Friday with a weekend rotation. **Employment Duration:** - Contract position for 13 weeks, starting September 15, 2025. **Why Join Us?** This is an excellent opportunity for a skilled healthcare professional to make a meaningful impact on patients' recovery journeys.
