**Job Title: Outpatient and Transitional Care Therapist** **Shift:** 08:00 AM - 05:00 PM **Schedule:** Sunday through Thursday **Start Dates:** 8/18 or 8/25 **Location:** Bryson City, NC **Position Overview:** We are seeking a skilled Outpatient and Transitional Care Therapist to join our team in a critical access hospital setting. This role requires a dedicated professional with experience in acute/transitional care, specifically lymphedema, pelvic health, and hand therapy. As a part of our healthcare team, you will provide high-quality patient care in both outpatient and transitional care units. **Key Responsibilities:** - Assess and evaluate patient needs in outpatient and transitional care settings. - Develop and implement individualized treatment plans based on patient assessments. - Collaborate with multidisciplinary teams to enhance patient care and outcomes. - Educate and support patients and their families regarding treatment options and care plans. - Contribute to the expansion of services within the facility. **Qualifications:** - Experience in outpatient and transitional care, with a focus on acute care, lymphedema, pelvic health, and hand therapy. - Current licensure and certifications relevant to the position. - Strong communication and interpersonal skills. - Ability to work collaboratively in a team-oriented environment. - Must be located 60 miles or more from the facility to be considered for the travel position. **Additional Information:** This position is part of a 48-bed, fully-accredited critical access hospital that has been serving the local community since 1948.
