**Job Title: Home Health Registered Nurse (RN)** **Shift:** Day 5x8-Hour (08:00 AM - 05:00 PM) **Weekend Requirement:** Every other weekend required **Job Description:** We are seeking a dedicated Home Health Registered Nurse (RN) with robust infusion and skilled home care experience. The ideal candidate will thrive in a fast-paced environment while providing high-quality care to patients in their homes. The role requires hands-on expertise in various medical procedures and the ability to manage complex clinical situations. **Key Responsibilities:** - Conduct comprehensive cardiopulmonary assessments and deliver thorough patient care. - Perform basic wound care, including ABI compression dressing, wound vac dressing, and staple/suture removal. - Administer intravenous (IV) therapy, including PICC line or Midline care and dressing changes, along with SQ port access and deaccess. - Manage Foley catheter care, laboratory draws, and vital sign assessments. - Deliver detailed instruction and support for home health procedures to patients and caregivers. - Collaborate with interdisciplinary teams including Physical Therapy, Social Services, and Pharmacy to ensure patient-centered care. **Required Skills and Qualifications:** - Minimum of 1 year of home health experience with OASIS training and experience. - Proven ability in managing high-risk diagnoses including Heart Failure, COPD, Myocardial Infarction, Pneumonia, and Stroke. - Proficient in phlebotomy and IV skills. - Familiarity with charting systems, preferably Cerner. - Ability to self-schedule home visits with a designated range of 6-8 visits per day. - Must possess a valid driver's license and a reliable vehicle for home visits. - COVID-19 vaccination required or exemption form on file, along with flu shot or declination. **Additional Information:** - Must be able to maintain compliance with facility requirements including masking standards. - Recent patient care experience (within the last 3 months) is mandatory. - Candidates must adhere to a 100-mile radius rule for employment eligibility.
