Job Description
**Job Title: Case Management RN – Day Shift (4x10)** **Overview:** We are seeking a dedicated and compassionate Registered Nurse (RN) for a Case Management position in an acute care facility. This role involves coordinating care within a multidisciplinary team and ensuring effective discharge planning for patients. The ideal candidate will have at least one year of experience in case management and a willingness to float between various departments within the hospital. **Shift Details:** - Schedule: 4x10-hour shifts (07:30 AM – 06:00 PM) - Start Date: ASAP - Weekends Required: Typically every other weekend **Responsibilities:** - Collaborate with a team of case managers to coordinate patient care across multiple units, including Emergency Department (ED), Intensive Care Unit (ICU), Pediatrics, and specialized units. - Conduct needs assessments and create individualized plans of care. - Facilitate discharge planning and ensure necessary follow-up services and equipment are in place. - Float between units as required based on patient care needs, including potential movement to the Pacific Campus (1.7 miles away). - Provide exceptional patient-centered care and advocate for patient needs. **Qualifications:** - Minimum of 1 year of experience in a similar case management role; first-timers accepted. - Experience in Emergency Department, Labor & Delivery, and NICU preferred, especially with pediatric patients. - Proficient in the EPIC electronic health record system. - Active nursing license in Washington state or Compact RN license. - Basic Life Support (BCLS) certification required. **Additional Information:** - Attire: Business casual - Orientation: 30 hours of orientation required - Local candidates not accepted; pending licenses are accepted. **Facility Highlights:** - Type of Facility: Level II Trauma Center - Total Staffed Beds: 571 across both campuses - Interdisciplinary support from Social Services available **Patient Care Details:** - Diverse patient age groups including neonates, adolescents, adults, and geriatrics.
