**Job Title: Licensed Practical Nurse (LPN) - Full Time Nights** **Job Description:** Are you a dedicated Licensed Practical Nurse (LPN) looking to make a meaningful impact in the lives of patients? We are currently seeking an experienced LPN to join our team for a full-time night shift position. As an LPN, you will provide comprehensive nursing care to adult and geriatric residents, ensuring that each patient receives individualized attention that reflects their specific needs and enhances their overall well-being. **Key Responsibilities:** - Provide both direct and indirect skilled nursing care to assigned residents. - Collect and monitor data related to residents’ physical and psychological conditions and communicate findings to Registered Nurses and interdisciplinary team members. - Participate in developing and implementing culturally relevant care plans that cater to the developmental stages and unique needs of residents. - Continually evaluate resident outcomes and suggest necessary amendments to care plans, ensuring all aspects of care are addressed. - Maintain a safe, healthy environment for residents while fostering a collaborative relationship with other healthcare staff, including physicians and ancillary professionals. - Stay current with nursing practices through ongoing education and training, including mentoring and orienting new staff. - Demonstrate compliance with all operational policies and protocols, keeping safety and quality care as top priorities. **Qualifications:** - Current New York LPN licensure. - BLS certification through AHA or RQI. - A minimum of 1 year of experience as an LPN, preferably in a Long-Term Care (LTC) facility. - Strong interpersonal and communication skills. - Ability to work weekends and holidays as required. **Behavioral Characteristics:** - Demonstrated leadership capabilities within a healthcare setting. - Ability to effectively collaborate with various healthcare practitioners. - Commitment to patient-centered care and customer satisfaction.
