**Job Title: Licensed Practical Nurse (LPN) - Woodbridge Rehab & Nursing** **Shift: 12-Hour Night Shift** **Job Description:** We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team at Woodbridge Rehabilitation and Nursing facility. As an LPN, you'll play a vital role in delivering quality patient care in a supportive environment. This position requires a commitment to team collaboration and a passion for working with elderly patients, particularly those with dementia. **Key Responsibilities:** - Provide high-quality nursing care to residents, ensuring their comfort and well-being. - Collaborate with healthcare teams to develop and implement individualized care plans. - Administer medications and treatments as prescribed by physicians. - Monitor patient conditions and report any changes to the nursing staff. - Assist with daily living activities and provide emotional support to residents. - Maintain accurate documentation of patient care and progress in line with facility standards. - Float to other units as required to meet staffing needs. **Requirements:** - Valid LPN license. - BLS certification through AHA is required. - Minimum of 3 years of clinical experience. - Previous travel nursing experience is preferred. - Long-term care (LTC) experience is essential. - Must be comfortable working with dementia patients. - Willingness to float to other units as necessary. - Must maintain flexibility with scheduling; no block schedules allowed. - Effective communication skills and a collaborative work ethic. **Additional Information:** - Background checks, including a nationwide search, FACIS3, and sex offender checks, must be completed prior to start. - Reliable transportation is recommended, as public transportation is not accessible in this area. - Candidates should be prepared for all-weather driving conditions from November to April. - All employment history information, including bed count and facility types, must be fully disclosed for submission. **Join our team and make a difference in the lives of our patients!
