**Job Title: Home Health Nurse** **Shift:** Day 5x8-Hour (08:00 AM - 05:00 PM) **Job Summary:** We are seeking a dedicated Home Health Nurse to provide high-quality patient care in a home setting. The ideal candidate will have extensive experience in home health and possess the necessary skills to manage a variety of patient diagnoses, including post-surgical recovery and chronic conditions. This role entails conducting skilled visits, creating home exercise programs, and ensuring compliance with safety and health regulations. **Key Responsibilities:** - Conduct patient assessments and evaluations for home care services. - Develop and implement personalized care plans based on patient needs and OASIS guidelines. - Provide education and resources to patients and their families about medications, diagnoses, and health management. - Collaborate with an interdisciplinary team, including physical therapy, social services, and pharmacy, to optimize patient care. - Document patient care activities accurately in compliance with policies. - Perform skilled nursing care including administration of medications, managing medical equipment, and monitoring vital signs. - Ensure patient safety and injury prevention, identifying and addressing any signs of physical, sexual, or psychological abuse. - Maintain a minimum of 6 home visits per day (potential for more at manager's discretion). **Qualifications:** - Minimum of 1 year of nursing experience in home health care. - Current BLS certification and valid state nursing license. - Experience with charting systems, Cerner experience preferred. - Strong assessment and documentation skills. - Knowledge of adaptive devices and mobility assistance protocols. - Valid driver’s license and access to a reliable vehicle for home visits. - COVID-19 vaccination required or a valid exemption form on file; Flu shot or declination required with mandatory mask wearing at all times while at work. **Skills Required:** - Proficient in the use of medical equipment and devices (e.g., gait belts, standard walkers, supplemental oxygen). - Familiarity with neurological and orthopedic conditions, chronic respiratory illnesses, and post-operative care. - Ability to manage time effectively and prioritize tasks in a fast-paced home environment. - Excellent communication skills for effective interaction with patients, families, and team members. **Additional Information:** - The position requires self-scheduling of visits and hours, based on the needs of the home care office. - Must reside at least 100 miles away from the facility for eligibility.
