**Job Description: RN Case Manager - Discharge Planning** **Position Summary:** We are seeking a dedicated and experienced RN Case Manager to join our dynamic team focused on discharge planning in an acute care setting. The ideal candidate will have a robust background in acute care management with a specific emphasis on discharge planning, and must possess proficiency with EPIC charting systems. **Key Responsibilities:** - Conduct thorough assessments for patients in the acute care setting to ensure appropriate discharge planning. - Collaborate with a multi-disciplinary team, including Social Workers and healthcare professionals, to facilitate smooth transitions of care. - Manage a caseload of approximately 15 to 20 patients per day, ensuring high standards of care and appropriate resource allocation. - Utilize naviHealth for referrals and stay updated with plans to transition to EPIC Care Management. - Consult with pharmacy and respiratory therapy teams as required to promote effective discharge planning. - Ensure compliance with discharge readiness criteria and advocate for patients needing complex placements. - Participate in departmental orientation and ongoing training. **Qualifications:** - Must be a licensed RN with at least 2 years of acute care management experience, specifically in discharge planning. - Mandatory experience must include a minimum of 6 months at a Level 1 or large academic medical center. - Proficiency in EPIC charting is a requirement. - General understanding of discharge readiness criteria is necessary; InterQual or Milliman experience is not required. - Certified Case Manager (CCM) certification is preferred but not mandatory; RN, BSN is preferred, but will consider ADN. - Current BLS certification is required. **Shift Details:** - Full-time positions available, working 5 shifts of 8 hours (Monday through Friday, 8:00 AM - 4:30 PM). - No weekend requirements, with holiday coverage rotated as needed. - Department-specific orientation will be provided, lasting approximately 2 weeks. **Work Environment:** - Work within a collaborative team comprised of 15 full-time and 17 part-time social workers and 15 case managers. - Engage with diverse patient populations, including complex cases, in an acute care setting.
