**Job Title: Home Health Physical Therapist** **Location:** York and Adams County, Pennsylvania **Schedule:** Full-time, Monday to Friday, 8:00 AM - 4:30 PM with every 6th weekend rotation. Open to 4x10 scheduling as well. **Overview:** We are seeking a dedicated and compassionate Home Health Physical Therapist to join our team in the York and Adams County region. The ideal candidate will have a minimum of one year of experience and will be responsible for delivering high-quality patient care in accordance with established clinical guidelines. This position involves working with adult and geriatric patients in a variety of home settings, and you will have the opportunity to make a meaningful impact on their recovery and wellbeing. **Key Responsibilities:** - Assess patients' physical needs and develop personalized treatment plans. - Provide therapy services, including exercise programs and manual therapy techniques. - Educate patients and families on care plans, home exercise programs, and adaptive equipment use. - Collaborate with an interdisciplinary team, including physicians, nurses, and rehabilitation staff. - Document patient progress and adjust treatment plans as necessary. - Stay informed about best practices and emerging therapies in home health. **Qualifications:** - Current PA license or compact license is required. - Preferred experience with OASIS documentation. - Previous charge experience is preferred. - Experience with Epic charting system strongly preferred. - Comprehensive knowledge of various patient conditions, including degenerative joint disease, COPD, stroke rehabilitation, and more. - Ability to work independently and manage a patient caseload effectively. - Professional attributes: excellent communication skills, strong problem-solving abilities, and a commitment to providing high-quality patient care. **Additional Information:** - The facility has a tobacco and fragrance-free policy; candidates should be notified before submission. - Care for COVID-19 patients may be required. **Why Choose Us?** At ARMStaffing we take care of our nurses!
