**Job Title: Registered Nurse (RN) - Patient Care Coordination** **Location:** Maryland **Shift:** 7:00 AM - 7:30 PM **Job Description:** We are seeking a dedicated and skilled Registered Nurse (RN) for a Patient Care Coordination role. The ideal candidate will be responsible for delivering high-quality nursing care, utilizing effective assessment and intervention techniques while ensuring optimal patient outcomes. **Key Responsibilities:** **Assessment:** - Compile patient databases and formulate nursing diagnoses. - Utilize physical assessment techniques and interviewing skills. - Anticipate the need for interventions by other health care disciplines. - Establish priorities for patient care needs or issues. - Reassess patients based on changes in their condition or treatment response. **Planning:** - Initiate and update written nursing care plans that reflect relevant scientific and psychosocial principles. - Integrate information from nursing care planning conferences into care plans. - Collaborate with patients, families, and health team members in planning hospital and post-discharge care. **Implementation:** - Independently perform clinical nursing skills. - Administer medications, IV therapies, and treatments accurately. - Initiate emergency procedures when necessary. - Delegate and coordinate tasks with ancillary personnel effectively. - Maintain thorough written documentation in patients' medical records. - Verbally communicate patient status and care plans to physicians and other health professionals. - Provide therapeutic communication with patients, families, and visitors, applying effective teaching and learning principles. **Patient Care Coordination:** - Manage daily patient assignments while setting appropriate priorities. - Monitor the nursing care performed by ancillary personnel. - Coordinate patient activities with various healthcare disciplines. **Teaching and Quality Improvement:** - Develop, document, and implement Patient Teaching and Discharge Planning Records. - Participate in patient, family, staff, and student education programs. - Engage in clinical monitoring and identify areas for improvement, addressing issues as needed or referring to appropriate resources. **Professional Excellence:** - May participate in RN 3 activities, excluding facilitation of the Nurse Residency Program. - Serve as a charge RN when competent, monitoring productivity and patient flow, and precepting new staff as needed. **Qualifications:** - Minimum of 2 years of experience with acute care or relevant nursing experience required. - Active RN license in Maryland. - Certification in ACLS (AHA), BLS (AHA), NIHSS, and others as required. - COVID-19 vaccination status to be included in the submission notes.
