**Job Title: Physical Therapist** **Position Summary:** We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team. As a Physical Therapist, you will be responsible for evaluating, planning, administering, and supervising Physical Therapy treatment according to physician referrals and established standards. Your role will involve providing high-quality, individualized care tailored to the unique needs of patients across various age groups. **Core Responsibilities:** 1. **Patient Evaluation and Care Planning**: - Assess patients' functional abilities and limitations to determine the need for intervention. - Create tailored plans of care with measurable treatment goals, prioritizing key limiting factors. - Collaborate with patients and their families/caregivers in establishing treatment objectives and initiating discharge planning. 2. **Implementation of Treatment Plans**: - Deliver individualized treatment considering the physical, social, spiritual, cultural, and educational needs of patients. - Continuously evaluate the effectiveness of the treatment and modify plans as necessary. - Educate patients, families, caregivers, and interdisciplinary team members regarding treatment and outcomes. 3. **Documentation and Compliance**: - Maintain accurate and comprehensive documentation in line with regulatory agency requirements and departmental policies. - Ensure documentation reflects evidence-based practices and detailed patient progress assessments. 4. **Communication and Collaboration**: - Effectively communicate vital patient information to facilitate coordinated care. - Provide clear updates on evaluation, treatment results, and patient progress to all relevant parties. 5. **Supervision and Mentorship**: - Supervise and delegate tasks to Physical Therapy Assistants (PTAs), Athletic Trainers (ATCs), and therapy technicians, while ensuring compliance with practice standards. - Aid in the training and orientation of new personnel to uphold quality care delivery. 6. **Departmental Activities**: - Attend mandatory department meetings and engage in process improvement initiatives. - Meet departmental productivity standards and contribute positively to team dynamics. **Minimum Qualifications**: - Graduation from an APTA accredited Physical Therapy program or equivalent. - A valid Physical Therapy license in the relevant state. - BLS certification required.
