**Job Title: Home Health Physical Therapist (PT) - West Portland, OR** **Shift:** Day 5x8-Hour (08:00 - 16:30) **Location:** West Portland, Oregon and surrounding areas (Newberg, Dundee, Willamina) **Job Description:** We are seeking a dedicated and skilled Home Health Physical Therapist (PT) to join our team. This role involves providing essential rehabilitation services to patients in the comfort of their homes. As a Home Health PT, you will work one-on-one with a diverse patient population, including adults and geriatrics, to help them achieve their rehabilitation goals. **Key Responsibilities:** - Deliver personalized physical therapy services to patients in their homes, focusing on recovery and wellness. - Conduct thorough assessments and create tailored treatment plans based on patients' individual needs. - Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. - Educate patients and their families on therapeutic exercises and home care strategies to support long-term recovery. - Maintain accurate and timely documentation of patient care and progress using the EPIC Remote Client for Home Health and Oasis tools. **Qualifications:** - Minimum of 1 year of skilled home health experience required. - Current Basic Life Support (BCLS) certification. - Familiarity with OASIS documentation is mandatory. - Strong communication and interpersonal skills to effectively interact with patients and their families. - Reliable transportation to travel to patient locations; occasional travel may extend up to a 60-mile radius. **Desired Skills:** - Experience using electronic charting systems, specifically EPIC. - Proficiency in provided patient care techniques, including but not limited to wound care, adaptive devices, and various therapeutic practices. - Ability to work independently and manage time effectively in a home health setting. **Additional Information:** - This position requires a flexible approach to patient care and the ability to drive to various points of service efficiently. - Weekends are not required, and there is no on-call duty. - Continuous professional development opportunities available. **Why Join Us?** At ARMStaffing we take care of our nurses!
