**Job Title: Integrated Case Manager for Population Health** **Job Summary:** We are seeking an experienced Integrated Case Manager for Population Health to join our patient-centered care team. This position plays a critical role in assessing, planning, facilitating, coordinating, evaluating, and advocating for comprehensive healthcare services for patients and their families. The successful candidate will address complex patient needs and ensure effective resource utilization to promote safety, high-quality care, and cost-effective outcomes. **Key Responsibilities:** - Conduct thorough assessments of patients and their families to identify biomedical, psychological, social, and functional needs. - Develop and implement personalized, patient-centered care plans to optimize care experiences. - Engage with patients and families through advocacy, communication, health education, and resource facilitation. - Utilize professional judgment, critical thinking, and motivational interviewing techniques to aid patients in overcoming barriers. - Provide support and counseling related to treatment decisions and end-of-life issues, including Advanced Care Planning. - Ensure seamless transitions between care settings by coordinating necessary services. - Advocate for appropriate service delivery within the patient’s health plan benefit structure. - Collaborate with the treatment team to co-manage patients with complex medical and social needs. - Continuously evaluate patient responses to care/treatment plans and make necessary adjustments. - Participate in process improvement activities aimed at enhancing organizational performance and safety. - Support interventions in sensitive cases involving child abuse, domestic violence, and elder abuse. - Assist with additional tasks and responsibilities as assigned. **Schedule:** This position requires daytime availability from 8 a.m. to 4:30 p.m., with rotating weekends. **Qualifications:** - Minimum of 5 years of inpatient case management experience (excluding telephonic, utilization management, or therapy experience). - Strong computer skills and experience with Epic EMR. - A valid and unrestricted Nursing license (RN) or Master of Social Work (MSW) with current licensure. - BLS certification through the American Heart Association is required. **Preferred Skills:** - Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care. - Knowledge of preventive service guidelines, behavior change theory, and case management principles. - Understanding of medical ethics, legal implications related to case management, and social determinants of health. - Strong organizational, planning, and implementation skills, with the ability to manage multiple complex patient needs. **Education:** - Required: Nursing degree or MSW. **At ARMStaffing we take care of our nurses!
