**Job Title: Care Manager (MSW or RN)** **Shift:** Day 5x8-Hour (08:00 - 16:30) **Location:** Swedish Edmonds Campus, providing remote assistance to the Providence Everett Colby Campus **Job Description:** We are seeking a dedicated Care Manager to join our healthcare team. The ideal candidate will be a Master’s level Social Worker (MSW) or a Registered Nurse (RN) with case management and discharge planning experience. This role is crucial in managing patient care and ensuring a smooth transition for patients from hospital to home or other care facilities. **Key Responsibilities:** - Coordinate care and services for patients, ensuring that individual needs are met through a comprehensive plan of care. - Conduct initial evaluations and develop personalized care plans, utilizing clinical criteria to ensure medical necessity. - Oversee discharge planning, including referrals to home care, rehab, and long-term care facilities. - Communicate effectively with multidisciplinary teams, patients, and caregivers, documenting all relevant communications accurately. - Maintain understanding of insurance coverage criteria and document patient and caregiver education effectively. - Utilize CMS guidelines, InterQual Criteria, and Milliman Guidelines for concurrent reviews, appeals, and continued stay reviews. **Qualifications:** - Master’s degree in Social Work (MSW) or Bachelor’s degree in Nursing (RN) with an Associate clinical license (or LCISW). - Active Washington state license or Compact license. - A minimum of 1 year of case management and discharge planning experience in an acute hospital setting. - Familiarity with charting systems such as Epic or Cerner. - Strong understanding of major disease processes, including dementia and stroke. - Knowledge of regulatory requirements and compliance standards in healthcare, including HIPAA and CMS guidelines. **Skills:** - Excellent communication and documentation abilities. - Competency in conducting biopsychosocial assessments and developing actionable care plans. - Ability to manage patient care coordination effectively and handle high-risk situations. **Working Conditions:** - Average daily census of 18-20 patients, with floating responsibilities to other facilities as needed. - Full-time position with potential for local candidates at travel rates.
