Job Description

This is a 13-week contract position, requiring Rehab Optima experience. Please ensure you have all necessary credentials and background checks completed as outlined.

Required Experience / Certifications / Licensure

• State License Verification
• Resume
• Three References
• County Criminal Check (7 years for all counties lived and worked)
• Federal Criminal Check (7 years or more)
• Multi-State Criminal Search (7 years or more)
• Nationwide Sex Offender Search (7 years or more)
• HHS/OIG Search
• GSA Search
• Social Security Number Trace
• 2 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
• SAM Search
• Maryland State Background Check
• CPR Certification (Fundamental Facilities)
• MMR Vaccination or Declination
• Hepatitis B Vaccination or Declination
• Tdap Vaccination or Declination
• Flu Shot or Declination
