**Job Title: Physical Therapist – Outpatient Rehabilitation** **Shift:** Days (0700-1730), No Weekends **Location:** Sweet Home, Oregon **Job Description:** We are seeking a dedicated and skilled Physical Therapist to join our outpatient rehabilitation team based in Sweet Home, Oregon. Our team focuses on evaluating and treating patients with injuries or conditions affecting physical mobility and functional independence. This is a rewarding opportunity to work in a diverse range of outpatient services, including but not limited to: - Sports and orthopedic rehabilitation - Pediatric rehabilitation - Neurological and stroke rehabilitation - Balance and dizziness rehabilitation - Wheelchair seating assessments - Neck, back, and extremity care **Key Responsibilities:** - Perform patient evaluations and develop personalized treatment plans - Implement therapeutic interventions to improve patient outcomes - Collaborate with a multidisciplinary team to enhance patient care - Document patient progress and modify treatment plans as necessary - Educate patients and their families about treatment protocols and self-care strategies **Qualifications:** - Current unencumbered Oregon Physical Therapy license is required - Healthcare Provider Level BLS certification required - Ability to travel to multiple points of care **Attributes We Value:** We are looking for a Physical Therapist who is passionate about helping patients regain their mobility and independence.
