**Job Title: Inpatient Social Worker - Case Management** **Location:** Portsmouth, NH **Shift:** 8:00 AM - 4:30 PM, Monday - Friday (No weekends) **Contract Duration:** 91 weeks **Positions Available:** 3 **Job Description:** We are seeking a dedicated Inpatient Social Worker to join our Case Management team. This role is crucial for providing high-quality patient care in a fast-paced hospital environment. The ideal candidate will have a minimum of two years of experience in a hospital setting and will be responsible for case management responsibilities, including discharge planning, care coordination, and utilization management. **Key Responsibilities:** - Collaborate with interdisciplinary teams to assess and plan for patient needs, ensuring effective care coordination. - Manage discharges, guardianship, and surrogacy planning while maintaining open communication with patients and their families. - Conduct assessments and develop discharge plans that consider long-term care placement when necessary. - Utilize the Expanse & Careport documentation systems for patient records and case management documentation. - Adhere to unit guidelines and policies to maintain standards of care. **Required Qualifications:** - Active Compact License (at submission) - Bachelor’s in Social Work (BSW) or higher - Experience with inpatient case management - Familiarity with discharge planning and long-term care placement - Competency in working with families to navigate care options - Strong communication and interpersonal skills - Ability to manage a patient ratio of 25-40 **Preferred Qualifications:** - Previous experience in a fast-paced hospital environment - Knowledge of utilization management and revenue integrity procedures **Work Environment:** - The Case Management team consists of 3 Social Workers and 10 RN Case Managers, supervised by a manager and a director. - The department emphasizes collaboration and supports staff through integrated resources. **Dress Code:** Business casual --- At ARMStaffing we take care of our nurses!
